The Quest for Better Breasts

Girls want them. Boys drool over them. Magazines worship them. Big boobs are kind of a big deal, and their reign shows no signs of deflating anytime soon. In this cleavage-obsessed society, bigger certainly seems to be better. In fact, breast augmentation was one of the most sought-after cosmetic surgery for US women in 2014, with 286,694 breast augmentations performed. Asian counterparts in China, India and Japan are sizing up in figures as well. Increasingly, more silicone gel implants are used instead of the saline cousin. Saline implants tend to feel like, well, a bag of water in the breast and may cause visible rippling in some cases.

However, in this day and age,  breast augmentation procedures have improved leaps and bounds; focusing not only on improving the size of breasts but also on correcting any asymmetry and residual sagging caused by age, childbirth and weight fluctuations to create the most ideal aesthetic outcome.

There are 3 primary reasons why one would seek out or consider breast augmentation.
  1. Reconstruction: the replacement of breast tissues damaged by trauma, disease, and failed or sub-par anatomic development, such as deformity due to an internal condition.
  2. Revision and reconstruction: to revise (correct) the outcome of a previous breast reconstruction surgery.
  3. Augmentation: to aesthetically enlarge the size of and enhance the feel and shape of the breasts

5th Generation Implants
5th generation silicone implants, aka gummy bear implants, were first introduced to the market in the mid-90s. The main benefit of silicone-gel vs. saline composition is the elimination of the possibility that the saline will leak into the body and the implant will find it’s way into another part of the body as well, causing health complications and costly surgical remedies, not to mention emotional trauma.

Silicone Vs Saline
Breast augmentation with silicone gel implants has evolved over half a century into a highly refined procedure that uses devices made with better technology and sophisticated surgical techniques. The new form-stable, anatomically shaped (“tear drop”) silicone gel breast implants produce more natural outcomes compared to round, smooth globes by respecting the normal contours of the female body. The only FDA approved 5th generation implant on the market is manufactured by Allergan.

When comparing the 5th to the 4th generation silicone implants, the difference is in the stability. 5th generation silicone maintains it’s shape; it will not conform to the space it’s in, like silly putty or memory foam, which is exactly what defines 4th generation silicone.

You may have heard or read that breast implants only last 10 to 15 years and they should be replaced thereafter. That would have been true for the 3rd and 4th generation implants but for the latest 5th generation breast implants, in general, they do not have to be changed if no issues occur as they are more resistant to rupture. However, you should know that the life of these implants varies according to the individual and the longer a woman has them, the greater the chances complications will develop, some of which require surgery.

Dangers of Using Cheap Implants

The French-made PIP implants which used unsafe industrial grade silicone (instead of the medical grade stuff that won it the CE mark originally) in Europe last year stirred a major silicone scare.

PIP breast implants were banned after it was revealed they contained cheap industrial silicone meant for use in mattresses rather than medical procedures. The substandard implants are between two and six times more likely to rupture than standard silicone breast implants. French health data, based on 17,000 surgical removals of PIP implants, reports a rupture rate of more than 25%. A key concern of women with PIP implants is that they will rupture and release cheap industrial silicone into their body, which may then have toxic, even cancer-causing effects. Women with the implants describe it as like living with a time bomb inside them.

What is the surgery like?
The surgery is a one-hour day surgery procedure performed under general anaesthesia. The post-anaesthesia recovery is usually fairly quick and simple. Once you have fully recovered from anaesthesia, you will be able to go home on your own.

What happens during the recovery period?
In general, patients will feel a bit sore and tight over the chest for several days after the procedure. There may also be some limitation of upper limb movements for the same duration of time. The breasts may also be swollen for a month or so. However, most patients are able to return to work a few days after the procedure. One can resume light gym activities after 1 month and full gym activities after 3 months.

A special recovery bra is required to be worn for 1-3 months after the procedure. This helps to keep the implants in place and alleviate swelling. After which, patients are allowed to return to wearing regular underwired bras.

What are the factors to consider before making a decision?
When embarking on this journey, you would need to spend some time evaluating why breast augmentation surgery is for you. Your personal motivation is the most crucial consideration in your decision-making process.

The next important factor you will need to be equipped with is patient education. Many patients start off by asking friends or family who has had prior experience. You can also find out more on the internet, although, your surgeon would be the best resource avenue for reliable and valuable information. He or she will be able to help you comprehend the benefits and risks associated with breast augmentation surgery and breast implants; along with the limitations that may be present due to your breast and body type.

Last but not least, you need to be aware that whilst wanting to feel great about your body, you need to hold a realistic expectation. You may already have in mind a particular shape and size of the breasts you would like to have. However, what you want may not be what is most appropriate for you. The right 'fit' would be one that balances and complements you body type, plus what your breast tissue will allow. These baseline considerations form your first step towards achieving the best possible result for yourself. This is why it is essential to have an honest discussion with your surgeon about what you want from this surgery: which brings us to the next significant factor - choosing a surgeon.

Choosing a surgeon
It is very important that the surgeon you are choosing is a certified plastic surgeon who has had experience performing breast augmentation surgery. Read up on his website and research on forums. You would be able to gather others' experiences on various plastic surgeons and breast augmentation surgery. When you are ready, make an appointment for a consultation with the plastic surgeon. Decide on the next step (scheduling surgery) only with a plastic surgeon who makes you feel comfortable.
